Sports Physical and ECG Screening at AdventHealth Training Center

We want your child to be healthy and able to compete. Available for all school sports and students under 18, this is a FREE event where your child can receive their sports physical and required ECG screening.
You must fill out a separate form for each child. Appointments can be scheduled throughout the time of the event and are on a first come, first serve basis. After registering, please monitor your email for information regarding your child's exam. A parent or guardian MUST be present during these exams.
